We recommend you review the PMBOK Guide or attend some PMP exam prep courses before you fill out your application form. This will help you understand what information the PMI considers most important.
Read the Code of Ethics and Professional Conduct (8pg.).
In fact, learn it - it will be tested in the PMP exam, in the form of situational questions that will ask you what you would do in a particular situation.
You will need agree to the Project Management Institute’s Code of Ethics and Professional Conduct before you can submit your form.

Be Truthful
As a general rule: Be truthful! In any information you will share with PMI, just be truthful as you should always be in life!
The PMI may choose to audit your application. If you get caught on a lie, there is a very good chance you will never be allowed to apply for any of the PMI certifications... ever.
So, just be honest, and professional and always act accordingly to the PMI Code of Ethics.
